If you do not have a physical form on file already, you do need to bring the original to the ASB window before the first practice. Do not try to show up right before practice on the first day and try to get cleared. The ASB window closes at 3:00 pm and that is when practice starts. You need to have a physical completed for Evergreen SD athletics in the last 2 years.
